Ukiah Krakens Aquatic Club Liability Waiver
In consideration of the opportunity for my child (the "Child"), to participate in the activities of the Ukiah Krakens Aquatic Club (the "Club"), I, the undersigned parent or guardian ("I"), hereby agree to the following:
Assumption of Risk: I understand that participating in the activities of the Club involves certain risks, including, but not limited to, the risk of physical injury or death. I voluntarily assume all such risks on behalf of the Child, and I agree to indemnify and hold the Club, its officers, directors, employees, agents, and representatives (collectively, the "Released Parties") harmless from any and all claims, damages, or liabilities arising out of or in connection with the Child's participation in the Club's activities.
Release and Indemnity: I hereby release the Released Parties from any and all claims, damages, or liabilities arising out of or in connection with the Child's participation in the Club's activities, regardless of whether such claims, damages, or liabilities are caused by the negligence or intentional acts of the Released Parties or any other person or entity. I further agree to indemnify and hold harmless the Released Parties from any and all claims, damages, or liabilities arising out of or in connection with the Child's participation in the Club's activities, including, but not limited to, claims, damages, or liabilities arising out of the negligence or intentional acts of any other participant in the Club's activities.
Medical Release: I authorize the Club to obtain medical care for the Child in the event of an injury or illness, and I agree to pay for any such medical care.
Acknowledgment: I have read and understand this waiver, and I agree to its terms and conditions on behalf of the Child. I have been given the opportunity to ask questions about this waiver, and all of my questions have been answered to my satisfaction.
