Ukiah Krakens Aquatic Club Photo Release Form

In consideration of the opportunity for me, or my child (the "Participant"), to participate in the activities of the Ukiah Krakens Aquatic Club (the "Club"), I, the undersigned parent or guardian ("I"), hereby grant the Club the right to photograph or film the Participant and to use such photographs or films in any and all media, including, but not limited to, print, online, and television. I agree that the Club may use such photographs or films for any purpose, including, but not limited to, advertising, promotion, and publicity.

I waive any and all claims I may have against the Club, its officers, directors, employees, agents, and representatives (collectively, the "Released Parties") arising out of or in connection with the use of such photographs or films. I further agree to indemnify and hold harmless the Released Parties from any and all claims, damages, or liabilities arising out of or in connection with the use of such photographs or films.

I have read and understand this waiver, and I agree to its terms and conditions. I have been given the opportunity to ask questions about this waiver, and all of my questions have been answered to my satisfaction.
